STATE OF NEW HAMPSHIRE

In the Year of Our Lord Two Thousand Five

AN ACT requiring health care providers to furnish a patient with a copy of his or her medical complaint form.

Be it Enacted by the Senate and House of Representatives in General Court convened:

1 New Section; Patient Complaint Forms. Amend RSA 332-I by inserting after section 2 the following new section:

332-I:3 Copy of Patient Complaint Forms Required. Health care providers shall provide a patient with a copy of his or her patient intake complaint form at the time of the medical examination.

2 Effective Date. This act shall take effect 60 days after its passage.
