Amendment to HB 1050-FN
Amend RSA 159-E:1, I(c)-(d) as inserted by section 2 of the bill by replacing it with the following:
(c) To request voluntary enrollment onto the list, or to request removal from the list, the applicant shall apply using forms prescribed by the department of safety. A person may submit a form requesting to be added to the list to the department of safety or through any licensed health care provider. The health care provider shall verify the person’s identity before accepting the form and may not accept a form from someone other than the person named on the form. By the end of the business day on which the form was received, the provider shall electronically submit the person’s completed form, including the authorization of the applicant, to the department of safety. The department of safety shall also make enrollment, removal, and any additional forms related to the process available on the department's website.
(d) Once listed, a listed person may apply for removal from the list.
