1 Vital Records Administration; Completion of Birth Worksheet. Amend RSA 5-C:19, II(a)(3)(A) to read as follows:

(A) Current name and maiden name .

2 Vital Records Administration; Completion of Birth Worksheet. Amend RSA 5-C:19, II(a)(6)(L) to read as follows:

(L) The Apgar score, which is an evaluation of a newborn infant's physical status, at one minute and at 5 minutes .

3 New Paragraph; Vital Records Administration; Completion of Birth Worksheet. Amend RSA 5-C:19 by inserting after paragraph XI the following new paragraph:

XII.(a) When the division of vital records administration releases information on a birth to the Centers for Disease Control (CDC), it shall not include:

(1) The names of the mother, father, or baby;

(2) The birth month and day of the mother or father;

(3) Their medical record numbers; or

(4) Their residential address, mailing address if different from the residential address, or zip code.

(b) Notwithstanding subparagraph (a)(2), the division may release the mother and father's birth year and the baby's birth date to the CDC.

4 Effective Date. This act shall take effect June 30, 2026.
