1 Fetal Death Reporting; Information Provided. Amend RSA 5-C:75, VII to read as follows:

VII. Information regarding the mother including her: full name, maiden surname , date of birth, residence, race, ancestry, education, occupation, and social security number.

2 New Section Vital Records Administration; Fetal Death; Limited Release of Information to CDC. Amend RSA 5-C by inserting after section 75-a the following new section:

5-C:75-b Limited Release of Information to CDC; Fetal Death.

I. When the division of vital records administration releases information on a fetal death to the Centers for Disease Control (CDC), it shall not include:

(a) The names of the mother or father;

(b) The birth month and day of the mother or father;

(c) Their medical record numbers; or

(d) Their residential address, mailing address if different from the residential address, or zip code.

II. The division may release the mother and father's birth year and the date of fetal delivery to the CDC.

3 Effective Date. This act shall take effect upon its passage.
